Lack of Commitment from the Top is the major factor, not only part-time Black Belts assigned but also part-time green belt, part-time leader and part-time team member. They cant focus on the project due to the load of daily job.

And never provide good training, they just want to make it fast, few days course and run the project

I would say lack of six sigma training and not understand the six sigma well is both make the six sigma fail in many company

For some consultants, Six Sigma is merely a set of quality management tools, statistical methodology and a form of employee certification that supposedly guarantees mastery of implementing Six Sigma principles. Though it is often the case that there is only really a rudimentary understanding of the tools and techniques involved that by itself, would be insufficient to drive real change. Even worse some consultants will try to train and certify your entire company, when only relatively few team-members ever really need to be to gain the best results. While most companies have attempted to implement some form of Six Sigma strategy, the actual bottom-line results in many cases have been disappointing.
I think reward and recognition system is part of HR policy. Anyway, this is the most important issue because employee won't see how they should learn six sigma.

To provides the desired services by means of Six Sigma, projects should be coordinated with the strategic objectives of the company. Democratic approach is not at all going to work over here.

Lean six sigma provides best techniques and process but will show no result if not practiced carefully.
